Day - 10 Hour (United States of America)

Job Summary

Provides Masters' level clinical social to address patient/family psychosocial adjustment to illness and post-hospital care. Provides information, referral, and consultative services to patients, families and hospital staff.

Population Served:
Acute medical and surgical patients including adolescent, young adult, middle-aged adults, geriatric. Units covered include the Emergency Department, medical, med-surg, ICU, Maternal-Child, NICU, Pediatrics.

Essential Functions

Specific duties and responsibilities may vary depending on department or program needs without changing nature or scope of this position or level of responsibility.

May be asked to perform other duties as assigned.

  • Coordinates with Case Manager and interdisciplinary team to assure appropriate action is taken to patient and family's psychosocial and discharge planning needs for discharge. Maintains ongoing communication with interdisciplinary staff and attending physicians to address these needs.
  • Understands and supports the rights of the patient/family. Facilitates conflict resolution and engages others in problemsolving discussions
  • Provides supportive counseling, grief counseling, crisis intervention and other appropriate therapeutic services to assist patients and families in coping with presenting symptoms and problems such as coping with illness, accepting medical recommendations, and loss of functioning. Provides education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.
  • Coordinates home health, skilled nursing, durable medical equipment, facility placement, including 5150's.
  • Participates as requested on the Hospital's Bioethics Committee.
  • Documents patient information per hospital and external regulatory requirements.
  • Facilitates appropriate planning for newborns with alternative discharge plans (i.e. adoption, CPS, police custody, etc.)
  • Identifies potential problems with current or new practice, peer performance, policies, functions, services or Risk Management issues and immediately communicates this to supervisors and actively participates in problem solving.
  • Identifies and makes provisions for pediatric and adolescent patients' academic education needs.
  • Employee must perform all duties and responsibilities in accordance with the C-ICARE Standards of the Hospital. C-ICARE is the foundation of Stanford Healthcare

Job Qualifications

Education

  • Master's in Social Work from accredited Social Work Program
Experience

  • Oneyear medical social work experience, prefer acute care.
License/ Certifications

  • Social work license (LCSW) preferred


  • BLS

  • Basic Life Support from the American Heart Association.
Knowledge, Skills, and Abilities

  • Demonstrated clinical skills with healthrelated issues.
  • Demonstrated discharge planning skills including planning for home health, durable medical equipment, skilled nursing and placement.
  • Ability to work independently and function effectively as a member of a multidisciplinary team.
  • Prolonged walking, sitting, standing; and prolonged phone use
  • Needs to be able to hear and see patients, read medical records and use computer.
  • Knowledge of available resources as well as federal and state medical assistance programs.
  • Strong understanding of the psychosocial needs of patients
  • Strong communication and interpersonal skills
  • Strong problemsolving skills
