    The Position of Community Support Specialist - Residential (Adult Program)

    We are seeking a dedicated Community Support Specialist for our Residential (Adult Program) department. The role involves facilitating mental health rehabilitation interventions and offering the necessary support to assist individuals with mental illnesses in achieving their rehabilitation, resilience, and recovery goals within our Thresholds operated residential program.

    Key Responsibilities:

    • Provide direct service to members through individual and group counseling, case management, job support, crisis intervention, and psycho-social rehabilitation services.
    • Assess member information, recommend treatments, and identify existing resources.
    • Develop relapse prevention plans and strategies.
    • Assist in creating Individual Treatment Plans (ITP).
    • Teach independent living skills and provide support with collateral sources.
    • Conduct comprehensive member assessments and crisis interventions when needed.
    • Support member self-advocacy and participation in decision-making processes.
    • Ensure service plans meet member needs and include achievable objectives.
    • Collaborate with hospital staff when necessary and monitor medication administration.
    • Contribute to maintaining a clean and safe residence environment for members.
    • Attend meetings, complete required trainings, and meet billing targets.
    • Use personal vehicle for work and comply with insurance coverage requirements.
    • Assist with purchasing groceries and meal preparation.
    • Participate in emergency plans and ensure member safety.

    Requirements:

    • High School diploma or GED
    • Preferred: Bachelor's degree in social work, counseling, psychology, or related field
    • Experience working with individuals with mental illness

    Skills/Certifications:

    • Valid Driver's License
    • Team player with strong interpersonal skills
    • Ability to document services in a timely manner
    • Trained in Basic First Aid and CPR
    • Ability to provide transportation for program members

    SCHEDULE: SAT & SUN 1PM-9:30PM (16 HOURS/WEEK)
