We are hiring a hospice-experienced LVN case manager. The position is a full time position, based in our office in Burbank.


Key requirements:


Knowledge of Hospice MD software

Care coordination experience

Assist our DON in developing and monitoring care plans

As a Hospice LVN Case Manager, you will play a pivotal role in providing compassionate care and support to patients and their families during one of life's most challenging periods.

You will be responsible for managing a caseload of hospice patients, coordinating care services, and ensuring the highest quality of life for patients in their end-of-life journey.

This position requires empathy, strong clinical skills, and the ability to collaborate effectively with interdisciplinary teams.


Pay:
$ $42.00 per hour

Expected hours: 40 per week
