Overview:

Patient Navigation Coordinator:


  • Full-Time, 40 hours per week.
  • Monday-Friday; Hours will vary.
  • Must be willing to travel onsite to Downers Grove, as needed.
The Patient Navigation Coordinator is the key link to patients.

This individual will coordinate with home paramedic teams in tracking visit requests, working closely with process improvement and partnering with the Medical Director.

The individual will need to maximize utilization and efficiency within the home paramedic program.


Responsibilities:


Post Acute/After a hospital discharge:


  • The role would need to match transition of care team members requests for home evaluations (patients deemed unable or unwilling to come in within 3 days post hospital discharge). This individual would then dispatch a paramedic to the patient's home to complete a telehealth visit with the patient's PCP, or if the PCP not available, from a pool of telehealth providers.
  • For patients who can not be reached by transition of care team, this role would dispatch a home paramedic for a "wellness check," to ensure the patient is okay. Those visits can then be converted to a telehealth visit if the patient is willing.
  • Coordinate hospital follow up visits by time and location to streamline the efficiency of the process and maximize visits completed per day.

Pre-acute calls:


  • For outpatients, this individual could receive calls from the contact center or MA concierge line of patients with acute complaints who are not able to come in to the office.
  • Coordinate with paramedics to dispatch to a patient's home within a few hours of call with an associated provider telehealth visit.

Home Annual Health Assessments:


  • In between or around post acute and preacute calls, this individual would schedule home annual physicals for home bound patients who are not able to come in to the office to see a provider.

Qualifications:

EDUCATION:


  • High school diploma or GED required.

EXPERIENCE:


  • 12 years Call Center or Customer Service environment required
  • 2 years experience in Medical Office setting
  • 12 years experience with Epic or EMR preferred
