The Geriatric Support Service Coordinator as a member of the interdisciplinary elder care team is an integral component of the community care and long-term care management system for elders.

They engage in activities, which incorporate a variety of disciplines and funding sources. The care manager is directly involved in the processes of assessment, service planning and coordination, advocacy, counseling, and gatekeeping. They engage in activities designed toward eliminating barriers and improving the conditions and well-being of all clients.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Be knowledgeable of and comply with all state, federal and agency regulations, policies and procedures pertaining to elder services and WestMass ElderCare's mission.
  • Completes initial intakes utilizing a comprehensive needs assessments on consumers generally 60 + years of age for eligibility, need, and appropriate services.
  • Develops a plan of care addressing individual needs of consumer and coordinates needed home services.
  • Provides ongoing outreach with area aging network.
  • Acts as CCA SCO GSSC Care manager for enrolled consumers and complies with CCA SCO documentation and assessment requirements.
  • Assists consumers and their families in identifying and securing appropriate services utilizing resources from the Home Care and/or other appropriate programs, existing community services, and other formal and informal supports.
  • In coordination and cooperation with the interdisciplinary care management team, develops a plan of care for designated individuals, effectively addressing their needs and taking into consideration all available funding and service resources.
  • Coordinates the delivery of services, linking the consumer to the full range of appropriate systems to meet their needs and comply with WMEC guidelines.
  • Completes, updates, and maintains comprehensive case records according to established procedures and time frames for each client in a caseload. This includes completing and keeping current consumer records in SIMS.

REQUIRED KNOWLEDGE SKILLS AND ABILITIES:


  • Possesses the ability and willingness to support the mission statement, goals and policies of WestMass ElderCare; and has the ability to interpret, understand and adhere to the regulations, standards, and procedures as set forth by the Agency and the Executive Office of Elder Affairs.
  • Possesses a Bachelor's Degree with preferred major in social work, human services, nursing, psychology, sociology or a related field.
  • Bachelor's Degree in another discipline from those stated above will have demonstrated experience and/or strong interest in the field of human services via previous employment, internship, volunteer activity and/or additional studies.
  • Of Hispanic cultural and linguistic background, with the ability to communicate effectively in English and Spanish (orally and written).
  • Possesses knowledge of the aging process, community resources and social health intervention techniques desirable.
  • Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check.
  • Valid driver's license required. Vehicle available during all working hours. Vehicle should be Registered and inspected in accordance w 540 CMR 4.00.

Pay:
From $24.00 per hour
