**Summary**
The Cincinnati, OH VA Medical Center is actively seeking a well-qualified Registered Nurse (RN) to function as a Patient Safety Manager. This position is located in the Quality Safety and Value (QSV) Department. This RN demonstrates leadership in delivering and improving patient care services through collaborative strategies with others.
** Salary**
$55,405 - $106,201 per year

+ **Duties**
The Patient Safety Manager provides direct staff support to medical center staff in planning, designing, integrating, and evaluating the effectiveness of the patient safety program in the facility. He/she has primary responsibility for the oversight of the facility-wide patient safety program. The Patient Safety Manager assists in directing staff within the facility towards process improvement that will support the reduction of medical errors.
This RN demonstrates leadership in delivering and improving patient care services through collaborative strategies with others. In addition, executes position responsibilities that demonstrate leadership, experience and creative approaches to the management of complex patient safety issues. This RN (Patient Safety Manager) is responsible for the Medical Center Patient Safety Program and for promoting the Culture of Safety within the organization.
Other duties include but are not limited to the following:
Assesses, plans, implements, coordinates and evaluates patient safety activities through a continuous performance improvement approach.
Collaborates with medical center leadership, Risk Management, and staff to proactively identify the potential for risk and to develop and implement remedies to prevent untoward events.
Coordinates and ensures timely follow-up and investigation of adverse events and near misses to facilitate the development and implementation of action plans designed to prevent re-occurrence.
Participates in organizational review processes, both clinical and administrative and actively participates in the systematic analysis of a wide variety of data.
Maintains a current knowledge of a wide variety of best practices and skills related to safety in health care and through assigned functions and tasks focused on improving safety and organizational performance.
Actively participates in unit/program improvement processes and initiatives as well as the customer service program
Must have experience and knowledge of data sources, data analysis, data display and use of statistical tools to enhance the interpretation and value of available data.
Knowledge of the processes for Root Cause Analysis, Electronic Patient Event Reporting System, National Patient Safety Goals, Patient Safety Alerts, Joint Commission Sentinel Events and FDA Safe Medical Device Act.
Demonstrates knowledge of and appropriate response to requirements, reviews and findings of regulatory agencies (i.e.: National Center for Patient Safety, The Joint Commission, the Office of Inspector General, VISN 10 Office, etc.)
Collaborates with multidisciplinary staff to identify performance topics, identify and define measurable criteria, track and report findings
Ensures processes for appropriate orientation, education, and continuing professional development standards and that these are implemented across disciplines
Fosters translation of new policies, procedures, research and standards into improving care
Provides leadership at a programmatic level for the evaluation of new policies, procedures, research and standards implemented.
The RN (Patient Safety Manager) reports directly to the Chief, Quality Safety and Value for daily operations.

** Qualifications**
**Basic Requirements**:
**United States Citizenship.**
**English Language Proficiency.** RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
**Licensure:** Must hold a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
**Education:** Must be a Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing () or The Commission on Collegiate Nursing Education (). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. **OR** The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) **OR** In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
**Grade Determinations**: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursin
