    Job Duties:
    The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments.

    The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.

    In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

    Qualifications:
    Prefer a bachelor's degree in psychology, social work, or a field related to mental health.
    Two (2) years of full-time experience or part-time equivalent in a mental health setting.
    Demonstrated knowledge of DSM-V and psychosocial rehabilitation and recovery principles.
    Experience in working with health care and mental health regulatory agencies.

    About the Campus:


    Crestwood's San Francisco Healing Center is a Mental Health Rehabilitation Center (MHRC) located in San Francisco and overlooking the beautiful Golden Gate Park.

    Pursuant to the San Francisco Fair Chance Ordinance, we will consider for employment qualified applicants with arrest and conviction records.

    The salary range listed below represents the minimum and maximum base pay per hour at the time of posting.

    Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
