Overview

The Admissions Director/Social Worker has the responsibility for the admission, discharge, and coordination of resident social services. This includes admissions, discharges, coordinating community services for residents, as well as assessing resident emotional and social work needs.

The Social Worker works with the interdisciplinary team to develop and implement a comprehensive plan of care for each resident.

What you will do

  • Coordinates the admission and discharge of residents with emphasis on a smooth transition for the resident and responsible party.
  • Documents all charting within the regulatory requirements.
  • Maintains records of all admission inquiries. Reports on data analyzes data for quality assurance purposes. Maintains an active waiting list for facility.
  • Coordinates mental health and other special services for residents.
  • Communicates with residents and the responsible party regarding room changes and resident needs.
  • Follows facility policy and procedures designed to promote resident and employee safety and reduce health care related injuries. Promotes employee compliance with these policies and procedures.
  • Participates in professional activities, inservice programs, and committee assignments to enhance personal, professional, and institutional advancement.
  • Participates in the development and implementation of the comprehensive plan of care.
  • Oversees the Admissions Department in order to meet regulatory compliance for admissions and social work.
  • Markets services to the community and potential referral sources with tours, phone calls, followup on referrals and contacting local resources for leads. Seeks out new referral sources within the local area.

Qualifications:

Education

  • Bachelors Degree, Social Work or Related Field (required)
  • Masters Degree, Social Work or Related Field (preferred)
Experience

  • 34 years social work or admissions experience in a health care setting (preferred)
  • 1 year supervisory experience (preferred)
