Case Manager - Homelessness Prevention (FT, Non-Exempt)


REPORTS TO:
HPS Supervisor


COMPENSATION:
$23.00/hr. to $27.00/hr. depending on skills and experiences


JOB SUMMARY:


ICAN, located in San Jose, CA, is looking for a bilingual and bicultural English/Spanish or English/Vietnamese Case Manager to support our HPS (Homelessness Prevention System) Program.

The HPS Case Manager will be working with low-income families to prevent or end homelessness by: 1) screening, assessing, and conducting intakes from prospective participants; 2) enroll families into the HPS program with all required documentations collected and completed; 3) provide enrolled families with temporary financial assistance; 4) provide families with strength-based case management to help them gain or maintain permanent housing; 5) connecting families to other community resources.

ESSENTIAL DUTIES & RESPONSIBILITIES:


_1)_
_Outreach and Engagement: (5%)_
a) Conducts client outreach at various community events

b) Establishes and maintains relationships with other community support agencies to network client services

c) Attends all required agency, staff, and community-based meetings and trainings as needed


_2)_
_Prescreening, assessment, and Intake: (10%)_
a) Conducts prescreens and intakes to determine participant eligibility and assess appropriate level of assistance needed.

b) Conducts HPAT interview and assessment to determine eligibility of HPS program

c) Collects and compiles necessary documents to verify eligibility and needs of client

Case Management:
(70%)_

a) Manages a flexible case load of up to 30 or more households, including households receiving one-time financial assistance and households participating in case management

b) Provides case management to individuals and families by establishing goals and strategies to achieve client's housing stability

c)

Provides referrals in order to secure necessary services and support for clients:
medical, Li-HEAP, job training, housing, Law Foundation, etc.

d) Assists families with finding and securing new housing, when needed

e) Assists families with immediate crisis prevention, advocacy, problem-solving and other appropriate interventions, as needed

f) Provides client follow up to maintain and monitor housing stability

g) Stay informed of community resources available for HPS clients

_Data Management and Quarterly Reporting (10%)_
a) Maintains case files and progress notes, adhering to professional standards and designated time frames

b) Maintains accurate, updated, confidential case files on ICAN's secured database

c) Documents program activity, including prescreens, intakes, financial assistance, and case management into HMIS within one to 3 business day of activity

d) Submits quarterly narrative, data and other reports as needed

REQUIRED EDUCATION & SKILLS**:


  • Fouryear degree in social work preferred or related degree with case management experience.

Bilingual and bicultural in English/Spanish or English/Vietnamese. English/Spanish or English/Vietnamese speaker, with ability to read and write in English/Spanish or English/Vietnamese.

  • Strong organizational skills; ability to plan, organize, prioritize and multitask. Ability to work with little direct supervision.

Strong communication skills, including the ability to deliver difficult messages with compassion and to communicate respectfully with families in stressful situations.

  • Ability to thrive in a flexible, fastpaced environment, while maintaining a positive, solutionoriented approach
  • Computer skills including proficiency in MS Word, Excel, PowerPoint, Google Suite, Adobe, and the ability to quickly learn and develop proficiency with tracking database systems

PREFERRED KNOWLEDGE & ABILITIES**:


  • Experience working with persons from diverse ethnic and cultural backgrounds who are economically disadvantaged.

OTHER REQUIREMENTS:


  • Must be able to lift 25 pounds as the service is sometimes physical. For example, setting up for events which may require transporting or lifting equipment and materials, e.g., easels, flip charts, refreshments, brochures, chairs, tables, etc.
