Description:The primary purpose of the
Hospital Liaison position is responsible for improving patient experiences and outcomes by facilitating effective communication, coordinating care, and offering support to patients and their families. Their work helps ensure that patients receive the care they need in a holistic and patient-centered manner.
Responsibilities for the Hospital Liaison:
- Serves as a liaison between hospitals, assisted living communities, physicians and other healthcare providers who provide referrals to facilities.
- Plans, develops and coordinates patient referrals, hospital & physician relationships.
- Secures new customers and works with facility leadership for both new and returning patients/residents to ensure a smooth admission into the facility.
- Actively communicates with facility leadership new admission information, projected changes in admissions, bed changes,
- Demonstrate a strong customer service approach through providing patient/resident and family members with all
- Make inpatient visits to patients and provide education to providers, patients and families that are a potential for
- Responsible for census development.
- Develop and implement public relations and marketing programs.
- Attend community events to increase census and to provide key community healthcare and organizations information regarding facility services.
- Works with various departments to ensure first impressions and standards are met for an overall professional presentation of the facility and services.
- Provides quality facility tours, maintains current knowledge of bed availability, ensures responses to all inquiries are handled properly, timely, and with appropriate followup.
Qualifications for the Hospital Liaison:
- Bachelor's Degree or Degree in Business Administration/Public Relations/Marketing or a related field (required)
- Extensive travel (required)
- Excellent verbal, organizational and communication skills
- Able to work independently with mínimal supervision.
- Must be able to maintain confidentiality of sensitive/confidential information.
- Strong computer skills (Microsoft Suite and all social media)
- Ability to work flexible hours, as admission responsibilities may dictate
- Demonstrated critical thinking skills, problemsolving abilities, effective communication skills, and time management skills.
