Our practices have achieved medical home certification, use a fully integrated Epic? electronic health record, and are focused on quality improvement and innovation in care delivery.
Team based care with embedded Pharm Ds, Behavioral Health Social Workers, telephonic Primary Care Social Work, RN Care Coordinators, Geriatricians and growing Advanced Practitioner partnerships.
