- Implement and oversee the health care plans for people supported
- Administer medications and perform medical procedures as needed

- Participate in complex case management and engage in continuous learning opportunities

As a Licensed Vocational Nurse (LVN) at Momentum, you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Adult Day Program (ADP).

Additional Information:
A $2,000 Sign-On Bonus* will be paid in two payments: the first payment after 90 days of employment and the second payment after 6 months of employment.
