Description

The purpose of a home health aide with Hudson Valley Hospice is to provide home health services to Hudson Valley Hospice patients and their families.

Guarantee that those services are consistent with the philosophy, goals, objectives, and policies of the Agency. We are looking for Certified Home Health Aides in both Ulster and Dutchess Counties.

As a Home Health Aide with Hudson Valley Hospice, you will be working with and providing care for those who are at the end of life.

It is crucial to have an understanding of hospice and what it means to provide ADL's for those at the end of life.


Pay rate starting at $19 per hour Looking for Home Health Aides in both Ulster and Dutchess counties No Mandating but overtime if wanted Hospice and Palliative Care Certification with salary increase

ESSENTIAL FUNCTIONS:


  • Completes Hospice Aide duties following established plan of care.
  • Provide 24hour continuous care for patient/families in their home, as needed.
  • Reports all patient condition changes and/or uncontrolled symptoms to primary nurse, oncall nurse, TL or DPS immediately.
  • Maintains adequate records of all patient/family care services, as per policy.
  • Report all cases of abuse/neglect to supervisor, as per policy.
  • Completes required competencies, infection control and insservices.
  • Assists with orientation of new Hospice Aides as requested by supervisor.
  • Participates in community education activities which promote Hudson Valley Hospice and its philosophy.
  • Notifies Hospice in the event of patient changes and uncontrolled symptoms.

WORK ENVIRONMENT/PHYSICAL DEMANDS:


The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.


  • Travel by car to patient's home; reliable transportation required.
  • Overtime required; occasional weekend coverage.
  • All shift availability flexibility to work crisiscare hours when needed.


  • Uniform

  • Scrubs required.
  • Ability to carry at least twentyfive pounds of equipment and/or supplies.
  • Moderate to high risk of exposure to unsafe environments.
  • Routine duties involve potential for direct exposure to blood or body fluids.

PREFERRED EDUCATION & EXPERIENCE:


  • New York State certified home health aide with a minimum of 12 hours per year of inservice education since receiving certification.
  • Willing to train CNA or PCA must have valid CNA or PCA and willing able to take HHA transition class upon hire
  • One (1) year of caring for terminally ill patients with a basic understanding of the hospice philosophy preferred.
  • Valid NYS driver's license with reliable transportation

Hours:
Various positions available in both Ulster and Dutchess counties - inquire about the specific schedule; 40 hours per week (all HHA's do provide one weekend day per month on call rotation)Pay:
starting at $19/hr
