Your Role


As a Value Based Care Manager, you engage primary care practices as the representative of Oasis to improve patient health and outcomes.

Navigating the health system can be complicated for everyone.

You will form deep relationships with providers to understand their challenges and create solutions to ensure patients can access the care they deserve.

Additional responsibilities:


  • Become a trusted partner to primary care providers (PCPs) and their clinical/office staff, serving as an extension of the practice in/around the community
  • Work with provider offices and the Oasis team to schedule and prepare for preventative care visits, as well as ensure appropriate followup care is scheduled
  • Support practices and clinicians with information and education regarding accurate HCC coding and documentation in conjunction with preventative care visits
  • Ensure patients remain actively engaged with their PCP
  • Support PCP with valuebased care activities including quality clinical documentation and maintenance of clinician and patient interactions within electronic health records
  • Help with appointment scheduling (if needed), transportation coordination, mail order prescriptions, referring to community resources, educating patients and arranging for durable medical equipment (DME)
  • Support, educate, and assist patients and their families to navigate and access their healthcare benefits and programs in partnership with practice staff

Requirements:


About You

  • Live in/near Richmond, VA
  • You are passionate about being part of a team to help identify the root cause of patients' conditions
  • You care about improving access to care. You provide the same level of commitment to patients that you would your own family
  • You have a healthcare background including working in a clinical environment
  • You may have a coding, quality, or Medicare Risk Adjustment background, giving you the expertise to help physicians properly diagnose conditions to improve clinical outcomes
  • You are a relationship oriented, high performing individual with strong interpersonal and organizational skills
  • You have at least two years of experience with demonstrated examples of service excellence
  • You are comfortable working independently to support PCPs, patients, and their families
  • You are energized by helping people and solving problems
  • You are prepared to travel within your community to meet providers and patients, largely in healthcare provider offices
  • You have demonstrated an ability to master and improve on administrative workflows
  • Your written and oral communication skills are strong
  • You enjoy working as part of a team
  • You have a track record of success in a remote work environment
  • You possess a High School Diploma/GED
