- Conduct clinical interventions and periodic direct care visits, and appropriately re-evaluate care visits.
- Provide continuous assessment and treatment of chronic, co-morbid conditions, as clinically necessary.
- Evaluate and treat residents for changes in their health status.
- Review medications, orders, and diagnostic tests, and respond to clinical abnormalities as required for all patients under their care.
- Collaborate with physicians as needed for comprehensive medical management.
- Perform Annual Wellness Visits for all long-term Nursing Facility residents and may conduct Annual History and Physicals as requested by Facilities.
- Assist in managing facility-wide illnesses through suitable collaborations.
- Work in coordination with other healthcare professionals like Therapists, Specialists, and Social Workers.
- Maintain regular communication with families and attend family conferences when feasible.
- Perform other duties as delegated.
- Valid license as a nurse practitioner or physician assistant in the operational state, along with current board certification in the respective field.
- Master's Degree in Nursing or Physician Assistant Studies.
- Preferably possess strong leadership, interpersonal, and problems-solving skills.
- Excellent written and verbal communication abilities are a must.
- Should exhibit traits of punctuality, organizational capabilities, and teamwork proficiency.
