We are in search of a Case Assistant to provide administrative assistance for our Community Transition Liaison Program (CTLP).

Responsibilities:

  • Support the Community Transition Liaison.
  • Accompany the Liaison during nursing facility visits as required.
  • Perform clerical tasks, such as data entry.
  • Ensure accurate case documentation and reporting.
  • Assist staff as necessary.

Requirements:

  • High School diploma or equivalent, Associates degree preferred.
  • Minimum of one year of relevant experience.
  • Exceptional written and verbal communication skills.
  • Proficiency in computer skills.
  • Ability to work with statistical and financial data.
  • Familiarity with general office procedures and equipment.
  • Proficient in Microsoft Office Suite or similar software.
