RESPONSIBILITIES:

  • Serves as a liaison with facilitybased quality committees and works with the interdisciplinary team to ensure compliance with core performance standards.
  • Utilizes information technology to obtain evidencebased practices and the latest research to provide current best practice information to care providers on the unit.
  • Assesses patient's response to treatment and care through other care provider information or examines patient directly (not on every patient)
  • Assists Director with monitoring of the unit's compliance with regulatory agencies requirements relative to patient safety including TJC, OSHA, CMS and others.
  • Provides input to nursing leadership on the evaluation and competency of nursing staff and ensures appropriate assessment of learner needs.
  • Ensures the effective education of the patient and patient's family to prepare for discharge.
  • Assumes a leadership role in the development, revision and implementation of clinical protocols which transition patients across the continuum of care.
  • Communicates constructive feedback to assist others in their growth as professionals and is cognizant of cultural differences when communicating.
  • Provides information to staff on nationally recommended safety practices and serves as a HRO Safety coach and champion. Collects data on patient outcomes on the unit i.e. falls, pressure ulcers, medication errors, etc.
  • The Clinical Nurse Leader (CNL) has the authority to intervene in patient care to ensure safety is paramount, patients and families unique needs are anticipated as well as their expectations are exceeded. The Clinical Nurse Leader impacts operations by ensuring clinical staff are supported in their delivery of quality care that is in alignment with the latest evidence based practices. The Clinical Nurse Leader is empowered to investigate and resolve patient care issues on behalf of unit leadership and may represent unit leadership in handling complaints and resolution of grievances.
  • Works with team members, other leaders and physicians to develop and trend performance improvement indicators based on national standards and best practices.
  • Writes or revises unitbased procedures, protocols to reflect latest evidence.
  • Consciously provides opportunities for less experienced nursing staff to observe and participate in physical assessment, data review and evaluation of individual outcomes provided by the Clinical Nurse Leader.
  • Oversees the management of patient care through assessment of patient population daily. Facilitates full team discussions including patient and family when ethical dilemmas arise.
  • Ensures effective communication through the continuum to support ongoing progress toward identified outcomes.

EDUCATION:


Required:
Master's Degree in Clinical Nurse Leader with CNL Certification


EXPERIENCE:


Required:
Two (2) or more years job related experience including working as a bedside nurse.

Preferred:

Five (5) or more years job related experience including working as a bedside nurse, nurse leader or Clinical Nurse Leader.
