ROLE AND RESPONSIBILITIES
A Care Coordinator assists with the treatment of patients who have various behavioral problems, such as substance abuse and mental health disorder.
The Care Coordinator position is expected to understand and aid in implementation individual treatment plans, monitor patient behavior and provide a safe, supportive environment for the patients.
Care Coordinators may provide and record patient medications along with assisting in daily activities, both therapeutic and recreational.- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients' records by reviewing case notes; logging events and progress.
- Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and postdischarge requirements; orienting and training family members; providing resources.
- Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Additional duties as assigned by Clinical and/or Executive Director
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- Education: High School Diploma or GED. Bachelors in related healthcare field preferred.
- 3+ years customer service experience, preferably in human services, possess demonstrated commitment to customer service.
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.

Pay:
$ $18.00 per hour
