Responsibilities:

• Conduct assessments to determine the needs of clients and develop individualized care plans
• Provide counseling and support to individuals with special needs,
• Collaborate with healthcare professionals to ensure comprehensive care for clients
• Assist in coordinating services and resources for clients, including medical documentation and case management
• Advocate for clients' rights and access to necessary services
• Collaborate with interdisciplinary teams to ensure effective communication and coordination of care

Skills:

• Strong knowledge of social work principles, theories, and practices
• Proficient in conducting patient assessments and developing care plans
• Familiarity with psychopharmacology and its impact on mental health
• Excellent communication skills, both verbal and written
• Ability to work effectively in a team environment
• Strong organizational skills and attention to detail
• Demonstrated ability to provide crisis intervention services

Qualifications:

• Bachelor's or Master's degree in Social Work or related field
• Valid licensure or certification as a Social Worker (if required by state regulations)
• Previous experience in a similar role is preferred but not required

$65, $70,650.00 per year
