TITLE:
Health Coach (part-time or full-time)


SUMMARY:

The Health Coach's goal is to support individuals in their health and wellness goals by empowering them with information, tools, and providing them with healthy foods to maintain and improve their well-being.

The Health Coach will report to the Senior Services and Nutrition Director and will be responsible for three main functions:


  • Implement health and wellness coaching with individuals 55+.
  • Coordinate chronic disease and nutrition education through health, chronic disease, and/or food/cooking/nutrition-related support groups and/or presentations based on need of health coaching participants and senior center members with LSH activities coordinator and center social worker.
  • Promote health coaching program and recruit health coaching participants with support of other LSH staff.

PRIMARY RESPONSIBILITIES

Health and Wellness Coaching:

  • Conduct wellness screen that identifies personal strengths and risks
  • Guide coaching participants through building an action plan that focuses on areas the participant chooses to work on
  • Coordinate with the participants' physicians as needed
  • Support participant through their action plan by offering ongoing encouragement, feedback, and monitoring. Other senior services staff will help with problem solving, health education, and referral to support groups and additional services, including individual and family counseling, as needed
  • Track participant and action plan information in program and organization databases
  • Work collaboratively with the center social worker to identify potential health coaching clients as well as coordinate needed services for health coaching clients
  • Work collaboratively with the activities coordinator to share information about educational and health topics of need/interest for coaching participants
  • Connect with local health and social services providers to make warm referrals and coordinate care
  • Support promotion of health and wellness programming through health fairs, flyers, and word of mouth
  • Partner with local health centers and hospitals for referrals and potentially collaborative activities
  • Other duties as assigned
EDUCATION/EXPERIENCE/SKILLS

  • A working knowledge of MS Office is required
  • Strong written and oral communication skills
  • Flexible schedule for occasional weekend and evening events and meetings
  • Ability to work with diverse groups of individuals
  • Strong problem solving skills, ability to juggle multiple tasks, while maintaining attention to details
  • Team oriented and high level of comfort asking for assistance from coworkers when needed
