Description

Responsibilities:

• Provide case management services to individuals and families in need
• Conduct assessments to determine client needs and develop individualized service plans
• Coordinate and monitor services provided by various agencies and organizations
• Advocate for clients' rights and access to resources
• Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure comprehensive care
• Provide crisis intervention and support during emergencies or difficult situations
• Assist clients in accessing and navigating the healthcare system, including Medicare and Medicaid
• Offer guidance and support in workforce development, including job search strategies and skills training
• Provide mentoring and supervision to clients to promote personal growth and development
• Assist with child welfare cases, including foster care placement and reunification efforts
• Address behavioral health issues, such as addiction counseling or mental health support
• Assist clients in obtaining housing assistance, such as Section 8 vouchers

Qualifications:

• Bachelor's degree in social work or a related field (Master's degree preferred)
• Previous experience in case management or a related field
• Knowledge of social service programs and resources
• Strong communication and interpersonal skills
• Ability to work effectively with diverse populations
• Excellent organizational skills and attention to detail
• Ability to handle crisis situations with sensitivity and professionalism

From $25.00 per hour
