The Clinical Manager - Hospice is responsible for the oversight and program management of the Clinical component of the hospice program and services in all settings (homes, facilities, hospitals) The Clinical Manager manages organization, direction and staffing according to hospice scope of services and patient/family needs. The Clinical Manager oversees the integration and standardization of care throughout Hospice services from the basis of Mission, Vision, Strategic priorities, Clinical Scope of Practice, Performance Improvement and Fiscal objectives --through coaching and teaching functions.
Requirements
· Education: Bachelors degree in nursing required.
· Licensure / Certification: Current valid license to practice Nursing in the State of Ohio. Hospice/Palliative Care Nursing Certification or ELNEC Certification preferred.
· Experience: Minimum of 5 years experience in hospice or hospital-based palliative care, with supervisory or Clinical Education experience required.
· Current vaild Ohio drivers license and functional transportation.
· Experience in staff development, education, and precepting nurses and other clinicians.
· Ability to analyze Performance Improvement findings to facilitate program change, improvement, and development of the IDT ( Interdisciplinary Team).
Responsibilities
· Develops staffing plans based on census, productivity, utilization of casual staff and the management system. Participates in development of strategic plan, including outreach and community education, and cooperates as manager to achieve predetermined cost containment and productivity outcomes.
· Assists with and facilitates employee concerns related to communication,performance, disciplinary action and other HR concerns as identified.
· Ensures financial performance and profitability through management of budgeted resources and understanding of department contracts in accordance with theFinance Department's standards.
· Collaborates with attending physicians, hospital staff, consultants, Palliative
· Physicians/Hospice Medical Directors, and Hospice Interdisciplinary Team regardingpatient care, treatment, services.
· Participates in interdisciplinary meetings/rounds so that care involving managementof complex, end-stage diseases; pain and symptom management; need for psychosocial/spiritual support; and caregiver support related to patient/family ability to provide care are provided according to established protocols. · Assists in development of policies, procedures and clinical guidelines.
· Verifies prior to implementation, physician/other licensed independent practitioner's orders.
· Administers medications, according to the organization's clinical and safety standards, policies, procedures, and/or guidelines
· Mentors safe and appropriate use of opiates and other analgesics adhering to guidelines related to physician-directed titration schedules.
· Initiates appropriate emergency procedures as per policy and in accordance with patient/surrogate informed consent.
· Reports and/or removes non-functional equipment
· Adheres to physician orders
· Identifies high-risk areas regarding patient/family satisfaction, safety and follows policies related to high risk/low volume/problem prone procedures.
· Communicates errors to manager so that remediation can be rapidly employed.
· Promptly reports high-risk situations as per policy.
· Collaborates with hospital and hospice educators in developing competency of hospice care staff in pain and symptom management
· Initiates appropriate referrals to chaplain, dietitian, pharmacist, consulting physicians, ancillary therapists and other services as per criteria and/or as authorized by Hospice Interdisciplinary Team.
· Communicates with patient, family, Acute Palliative Care and HospiceInter-disciplinary Teams and other health care team members about pertinent clinical information, end-of-life issues, and potential financial implications of patient/family decisions.
· Meets population specific and all other competencies according to department requirements.
· Promotes a Culture of Safety by adhering to policy, procedures and plans that are in place to prevent workplace injury, violence or adverse outcome to associates and patients.
· Manages the educational priorities and activities that promote professional competence and personal growth of all Hospice team members.
