JOB SUMMARY


THE NURSE CASE MANAGER WILL BE RESPONSIBLE FOR SCHEDULING CAREGIVERS AND ENSURING EFFECTIVE CARE COORDINATION FOR A CASELOAD OF CLIENTELE.


ROLE AND RESPONSIBILITIES

  • Be the champion of change for the caregivers and ensure they feel and understand that they are the most valuable asset to Family Tree
  • Develop lasting relationships with caregivers and ensure that caregivers are fully prepared to succeed at their assignments and drive client satisfaction.
  • Ensure that clientcaregiver match is the "right fit" by scheduling an appropriate skilled person who is the best match for fulfilling a client's needs.
  • Mentor, coach, and reward caregivers to reach their full potential.
  • Attend weekly meetings with supervisor and other care coordinators to discuss current clientele and staffing needs.
  • Review care plan for new clients and put unassigned schedule into ClearCare to ensure monitoring and tracking of staffing progress.
  • Negotiate pay rates within established parameters based on the difficulty of cases, challenging locations, and urgency of service as appropriate.
  • Assign pay rates and bonuses for caregivers (per approval by Caregiving Services Manager) within the parameters set through company policy.
  • Perform regular (minimum weekly) checkin calls with caregivers to remind caregivers of expectations as needed in regard to dress, conduct, timeliness, etc.
  • Document check in calls with caregivers at least 24 hours after starting a case to ensure appropriate tasks have been completed, asses the case, and escalate any client concerns (or nonscheduling related concerns) as needed to the appropriate care advisor.
  • Maintain documentation in ClearCare of any communication with caregivers related to client care, including personnel problems or situations that arise with caregivers and how the situation was handled (e.g. documentation for absences and callouts).
  • Communicate caregiving staffing in regard to punctuality, reliability, and performance in collaboration with division supervisor (per policies and procedures set forth by HR).

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • High school diploma required.
  • At least 2 years of scheduling experience in home care required.
  • Experience in ClearCare preferred.

REQUIRED SKILLS

  • Excellent communication and interpersonal skills.
  • Excellent organizational skills and attention to detail.
  • Thorough understanding of the home care business.
  • Strong analytical and problemsolving skills.

PHYSICAL REQUIREMENTS

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift to 15 pounds at times.
