    Summary

    The incumbent manages the facility Veterans' Health Education and Information (VHEI) Program. This results in an evidence-based, Veteran-Centered facility VHEI Program that supports facility goals and objectives, and meets the needs of Veterans, their family members/caregivers, and clinical staff, while enhancing their quality of life through health promotion and disease prevention.

    Duties

    The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.

    Duties include but are not limited to the following:
  • Functions as the designated focal point, coordinator, advocate and liaison for all Veterans and family health education programs, and activities; serves as the facility content expert in health education. Functions as a focal point, collaborator, coordinator, advocate for Whole Health programs including self-management and Complimentary and Integrative Health (CIH) programs.
  • Functions as a designated focal point, coordinator, advocate and liaison for Community patient education, wellbeing, self-management and CIH programs.
  • Develops partnerships with community programs for patient education, self-management and CIH programs, including developing referral processes and participation tracking.
  • Works with facility management to assure that the Veterans Health Education Program contributes to the facility's mission, goals and objectives.
  • Demonstrates leadership, experience, and creative approaches to the management, delivery and maintenance of the Veterans Health Education Program.
  • Provides consultation and training to interdisciplinary staff members regarding health education theory and practice, evidence-based health education interventions and skills, and health education program development to assure that Veterans health education programs, activities, and services meet quality standards.
  • Serves as the principal point of contact for all Veterans health education communications and reporting within the facility, among the facilities in the Veterans Integrated Service Network (VISN), with the Office of Veterans Health Education and Information (VHEI) and other program offices.
  • Participates in the establishment and review of pertinent policies and procedures
  • Chairs the facility's interdisciplinary Veterans Health Education committee to assure interdisciplinary collaboration in the design and development of health education programs, activities, educational materials and services for Veterans and family members
  • Assures that the Veteran Health Education Program has met all VHA Veterans Health Education Program requirements and accreditation standards related to patient education
  • Leads, review, vetting and approval of in-house created Veteran Health Education materials
  • Leads the Veteran's Health Education strategic planning process
  • Provides consultation and technical assistance to other program areas to integrate Veterans health education across the continuum of care within the facility
  • Provides consultation and training to interdisciplinary staff members to enhance their understanding and practice of evidence-based health education interventions and skills, and health education program development
