    Description

    An esteemed community center dedicated to providing expert social services to individuals and families, spanning children, youth, and adults is currently seeking Case Managers for its Manhattan location.

    This individual will collaborate with the social service team to deliver case management services to formerly homeless adults diagnosed with severe persistent mental illness, supporting them in maintaining permanent housing.

    The actual compensation offered to the successful candidate may vary from the posted hiring range based on geographic location, work experience, education, and/or skill level, among other things.

    This is a temp-to-perm role; Offer case management services to a designated client group, pinpointing both short and long-term treatment objectives and assisting clients in reaching them.

    This involves connecting clients with community programs like medical, psychiatric, and substance abuse treatment services.

    • Additionally, coordinate and monitor interactions with medical and psychiatric treatment providers.
    • Attend weekly staff meetings, case conferences, training sessions, and supervision meetings.
    • Perform any additional responsibilities as necessary in alignment with agency policies and funding source regulations, such as incident reporting, data gathering, and preparing for audits.
