DESCRIPTION:
This position involves the provision of an array of counseling and educational services within the home and community setting.
Counselors are responsible for all client related documentation and reports as well as the development and implementation of treatment goals and objectives.
Facilitating parent and peer support groups as needed is also expected.Counselors are required to provide their own transportation with reimbursements given on a monthly basis for use of their vehicles on a mileage basis_.

On a monthly basis, the vast majority of the services provided are to be in the form of direct involvement with client' and/or their family members.
With this direct care 75% of the services are delivered in the school setting, with remaining 25% of care to be given in home._ Remaining hours will consist of conducting service coordination and administrative duties.
Position reports directly to DOS.
TASKS:
1) Participate in the initial thirty-day assessment of client/family situations and ongoing monthly reports following the approved format.
Provide a service plan within the initial thirty-day period, revising as needed thereafter to reflect client progress and confirmed needs.
2) Monitor length of client services and notify referral sources prior to the completion of ninety-day period to determine if continuation is necessary.3) Act as the liaison between family, referring agencies, and other community organizations.
4) Coordinate client services between various agencies.
5) Maintain case records and document services rendered.
6) Attend weekly staff meetings consistently. Prior approval from management team is required when absent.
7) Prior to the end of each thirty-day service period, coordinate and facilitate a staffing for the client, inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision.
8) Maintain high degree of client confidentiality.
9) Work with other staff assigned to cases, communicating effectively.
10) Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.
11) Provide Individual and Family counseling services to clients in the home, school and community setting.
12) Provide Crisis Intervention as needed.

MINIMUM QUALIFICATION:
A Bachelor's of Science degree in a human services field and a minimum of 3 years of successful clinical work experience (direct services to individuals with a mental illness, intellectual disability, receiving gerontology or special education services) with children / adolescence in the field of institutional management, social work, education or other allied discipline or mental health agencies.
