    Bureau Description:

    The Bureau of Chronic Disease Prevention works to prevent and reduce the impact of chronic diseases and disabling conditions by addressing associated modifiable risk factors.

    Chronic diseases including heart disease, stroke, cancer, diabetes, and asthma are among the most common, costly, and preventable of all health problems in Florida and in the U.S.

    The Bureau develops, implements, and evaluates health promotion activities, primary and secondary prevention services, and community-based health interventions across the state.

    The Bureau partners with state and local agencies, health systems, and community partners to encourage best practices statewide. The Bureau also promotes the inclusion of persons with disabilities in all program activities. The Bureau supports community and state-level partnerships including activities that promote the Department's Healthy Communities of Florida Program.

    These partnerships ensure coordination and collaboration among and between stakeholders and providers to stimulate and support efficient and effective delivery of health care.


    Your Specific Responsibilities:

    This is a highly responsible professional position under the direct supervision of the Health Systems Section Administrator in the Bureau of Chronic Disease Prevention, Division of Community Health Promotion.

    This position requires advanced analytical skills involving planning, evaluation, monitoring, and reporting of statewide projects; networking with other program offices and government agencies; excellent communication skills; organizational management skills; and the ability to work with considerable independence.

    Duties include, but are not limited to the following:


    • Design and manage projects to increase referrals to, use of, and reimbursement for diabetes prevention programs (DPPs) that meet standards established by the Centers for Disease Control and Prevention's (CDC's) Diabetes Prevention Recognition Program. Project design includes but is not limited to developing budgets, implementation milestones, measurable objectives, and evaluation criteria to document project outcomes.
    • Design and manage projects to increase referrals to, use of, and reimbursement for diabetes self-management education (DSME) programs that meet the National Standards for Diabetes Self-Management Education and Support established by the American Diabetes Association, American Association of Diabetes Educators, and other members of the DSME Taskforce. Project design includes but is not limited to developing budgets, implementation milestones, measurable objectives, and evaluation criteria to document project outcomes.
    • Assist with grant application completion and submission of reports to the CDC.
    • Facilitate support for the Diabetes Advisory Council (DAC), including coordination of legislatively mandated reports and meetings.
    • Assist in procuring services and materials by preparing appropriate procurement documents (ITN, RFA, ITB) through MyFloridaMarketPlace (MFMP) and negotiating with potential vendors. Follows Department and MFMP procurement procedures, processes invoices, tracks payments, troubleshoots problems, and monitors payment and encumbrance histories in all necessary financial systems.
    • Develop written materials in response to special projects and assignments related to chronic disease prevention and health promotion for the Bureau, Division, and Department and participate in educational opportunities to enhance professional development.
    • Performs other duties and responsibilities as assigned or required by bureau program activities.

    • Knowledge of public health care system, quality management practices, and chronic disease prevention and control.
    • Knowledge of public health programs, management principles, policies, laws, functions, and practices.
    • Ability to develop and write policy, rules, protocols, procedures, manuals, guidelines, grants, and reports.
    • Ability to communicate effectively verbally and in writing.
    • Ability to work collaboratively with others and establish effective working relationships with a wide variety of groups.
    • Ability to develop educational materials, and plan and conduct trainings.
    • Utilizes computer or microcomputer to prepare correspondence, reports, and other work.

    Qualifications:

    Education:

    Preferred candidate will possess a master's degree in public health or similar field with at least two or more years of public health experience.
