Experience

• Provides on-site, team-oriented primary care to patients within Assisted Living and Memory Care facilities 2 days a week
• Approximately 12-16 patients per/day
• Develops and implement care plans appropriate to patients and their desires
• Initiates, monitors, and alters medications and treatments within approved guidelines and policies and procedures
• Initiates diagnostic laboratory test and interprets and evaluates laboratory results
