Housing Navigator


POSITION TYPE:
Case Manager - Full Time Non-Exempt


DESCRIPTION:

The Housing Navigator - Families is responsible for working with homeless families and individuals to develop a personalized case management plan that addresses their barriers to returning to housing.

This includes benefits and health care advocacy, budgeting assistance, short
- and long-term planning for housing and employment search, education advocacy and connection to other services as needed.


REPORTS TO:
CES Manager


SALARY:
$22.00 hr.


Duties and Responsibilities:
Conduct initial Access Center and Emergency Housing Program intakes and develop individualized services plans with families

Maintain Client Records o Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines; o Maintain client related data systems, including case notes and HMIS entries; o Prepare case management related reports including but not limited to: outcomes, successes, etc;
Client Follow-up o Provide housing counseling and coordinate moves to permanent housing o Continually monitor and evaluate each client's progression through their housing case management plan, and develop corrective action revisions to the plan as needed;
Research and update client resources o Attend program-related community, coalition and committee meetings as assigned; o Maintain up-to-date records of providers and resources

Case Conferencing o Ensure effective service delivery by coordinating work with Ascencia as well as other staff as appropriate.

Other duties as assigned.

Qualifications and Requirements for this Position: 4 year degree in human services or comparable combination of education/work related experience required;
Experience in social service setting with working knowledge of case management systems and planning techniques; familiarity with HMIS system

Knowledge of homeless and housing related resources, housing rights issues, and the Continuum of Care for homeless persons preferred;
Experience working with homeless individuals and families in coordination with the Coordinated Entry System and
Family Solutions Centers preferred;

Must be insurable and provide own transportation for work related travel as necessary.

Must possess a valid driver license;

Employees must carry active and adequate auto insurance corresponding to regulatory requirements if driving any vehicle not owned or leased by Ascencia.


Physical Working Conditions and Office Setting Description

Some travel required to field locations to facilitate execution of responsibilities. Must maintain a current driver's license and valid auto insurance with a minimum of state required levels

Requires prolonged sitting, standing, frequent bending, stooping and stretching

Some lifting of up to 25 pounds may be required
-
Frequent and prolonged typing, as well as operation of a computer. Use of keyboards and phones required

Pay:
From $22.00 per hour
