Requirement:


  • Minimum 12 years of LPN Case manager experience required.
  • Must have experience in authorizations and referrals and case management more so than clinical.
  • BLS required
  • Current California LVN license.
  • High School Diploma or GED required.
  • Graduate of an accredited LVN Program. A.A., Diploma preferred.
  • Identifies, plans, and coordinates highrisk patient care (frail, chronic disease, catastrophic injury) in an ambulatory setting. Ensures continuity of care across the continuum. Promotes and evaluates quality and costeffective outcomes.
  • 6 months assignment (Extendable)

Pay:
$42.00 per hour

Expected hours: 40 per week
