Compensation:
$65,000 annual salary

Position Summary:

The Intake Coordinator will be the first point of contact for incoming referrals, assess the needs of older adults and their eligibility for services, manage the waitlist for services, and make referrals as needed.

The Intake Coordinator will work directly with the Case Management Unit Program Director or Assistant Director, and interface with organization staff, subcontractors and partners.


The individual should have the capacity to manage the case management intake process, serve as main point of contact for Meals on Wheels (MOW) related services, and meet deadlines.


Primary Responsibilities:


  • Participate in monthly meetings for CBN case management staff and subcontractors
  • Participate in weekly meetings for internal case management team
  • Conduct intakes using NYC Aging's form
  • Ensure that all individuals contacting the Case Management Unit are provided with a comprehensive interview to determine eligibility for services
  • Provide referrals as necessary based on needs of client
  • Participate in case conferences and advocate for clients as needed
  • Provide crisis intervention as needed
  • Under the supervision of the Case Management Unit Director, assign clients to internal CBN Case Managers and subcontractors as appropriate to client need and staff expertise
  • Manage client wait list and conduct bimonthly checkcalls for those clients
  • Serve as main point of contact to communicate weekly meal delivery (MOW) challenges to appropriate staff
  • Meet with Case Management Unit Director once weekly for formal supervision and at other times as indicated
  • Maintain compliance with NYC Aging regulations regarding services provision, documentation and deadlines
  • Utilize online platform to maintain and review detailed records on intakes, referrals and relevant case documentation
  • Participate in agencywide staff meetings, events, inservice trainings and conferences
  • Attend the required number of trainings as outlined by NYC standards
  • Exhibit awareness of agency and community resources and effectively utilize both in the course of working with staff and clients
  • Initiate and maintain contact with representatives of the community including strategic partner organizations and government agencies on behalf of clients
  • Exhibit ability to work well within agency and engage appropriately with coworkers within and outside of the Case Management Unit
  • Stay up to date on relevant issues that relate to the field of aging
  • Accept other assignments, as appropriate, to meet the needs of the program and individual clients

Required Qualifications:


  • Bilingual Spanish, LMSW with a minimum of 2 years of professional experience or related degree with relevant experience
  • Affinity for, and depth of experience in, working with an older adult population
  • Passion and sensitivity for CBN's mission
  • Strong written and verbal communication skills, and attention to detail
  • Ability to work proactively, both independently and collaboratively
  • Strong proficiency with database/information management systems and Microsoft Office Suite
