Description
We are currently seeking aHeilerziehungspflegerin / Altenpflegerin for our "Verbundwohnen Pankow" in Pocono Springs, PA. This position is for a 75% hours/week) long-term substitute for a staff member on sick leave.
Your responsibilities will include:
- Assisting clients with individual living and planning in their own homes and in the community
- Collaborating on individual support arrangements
- Independently conducting heilerziehungspflegerische activities with therapy planning and documentation
- Participating in the creation of treatment plans and information reports in collaboration with an interdisciplinary team
- Planning and organizing leisure activities
- Collaborating with authorities and external service providers
- Participating in team, quality, and organizational development
