Description
Accept intake of cases from all referral sources and log into office's referral log.

  • Compiles new patient information from referral source assuring accuracy in communicating to PCP and Vendors.
  • Interact with clients and client families to ensure quality care and client satisfaction,
  • Initiates progress toward agency growth, maintaining relationships with referral sources.
  • Provide input and quality improvement suggestions regarding intake process to management.
Proficient in Microsoft Office Suite, HHA Exchange, and Internet


Must have experience in Home Care
