GENERAL SUMMARY:


Comprehensive Community Services (CCS) is a recovery-focused behavioral health program for adults with severe mental illness &/or substance use disorders.

CCS is a community-based program in which the majority of services are provided in clients' homes & communities.

The program is person-centered & uses client-directed service plans to describe the individualized services that will support the consumer in achieving their recovery goals.

Services are provided by teams of professionals, peer specialists, & natural supports which are coordinated by a CCS Care Coordinator.


Care Coordinators help develop skills for independence, leadership, confidence, and empower individuals to take ownership of their lives and achieve their goals.


ESSENTIAL FUNCTIONS:


  • Provide care coordination services to each client throughout the duration of enrollment in CCS services.
  • Accepts referrals of eligible consumers/clients and initiates the process to assess strengths and needs in a timely manner.
  • Facilitate team meetings to develop a care plan based on strengths, values, and preferences.
  • Educate consumers/clients on how to utilize community supports, both formal and informal.
  • Assist the team in establishing measurable consume/client outcomes and reduce any system barriers that may arise.
  • Documents consumer/client plans, collects consumer/client outcome data, and prepares all documentation according to program required time frames.
  • Facilitates annual Functional Screens to ensure consumers/clients meet eligibility requirements.
  • Carries a caseload ranging between 1518 depending on needs of consumers/clients.
  • Attend training and ongoing staff development opportunities.
  • Adheres to St. Charles Youth & Family Services Policies and Procedures.
  • Other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • Bachelor's degree required; Master's degree preferred in a relevant area of education or human services.
  • 1 year experience working in a mental health related setting.
  • Knowledge base of individual and family issues and dynamics.
  • Ability to prepare formal reports.
  • Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
  • Strong verbal and written communication skills.
  • Valid Wisconsin driver's license and liability insurance.
  • Ability to use vehicles for work related duties according to the agency's insurance liability standards.
  • Ability to accurately perform record keeping and documentation entries and do so within the required time frame.

WORKING CONDITIONS

  • General Office Environment
  • Community and in home setting during client visits.
  • Travel approximately 70% using a company vehicle.
  • Potential for exposure to blood products, contagious disease, etc.
