Summary:

Community Health Workers, who work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports.

This position is with NMIC but works in close partnership with New York-Presbyterian (NYP) Hospital and reports to two managers, one at NMIC and one at NYP.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Enrolls and manage caseload of patients who meet rising risk program criteria
  • Conduct at least 2 home visits and provide appointment accompaniment as needed.
  • Offer practicebased support and information sessions at an assigned patient centered medical home, CBO or hospital. Collaborate with the healthcare team throughout the patient centered medical home and hospital.
  • Helps patients enroll onto/navigate patient portal
  • Supports patients to navigate the health care system
  • Completes required documentation associated with assessments and educational sessions using approved collection method.
  • Maintain an organized filing system and submit all paperwork and documentation to program staff by designated deadlines.
  • Facilitate/host two communitybased workshops per year.
  • Conduct communitywide outreach and assist with event planning, including workshops and health fairs.
  • Make social services and other referrals to address social determinants of health.
  • Attend all required staff meetings and trainings at both NYP and NMIC.
  • Additional related responsibilities may be assigned.

QUALIFICATIONS:

  • 2+ years of CHW experience or comparable education/training and experience strongly preferred.
  • Experience facilitating workshops and providing oneonone health support and information is preferred.
  • Willingness to visit with families in the hospital and in the community (comfortable conducting home visits).
  • Strong interpersonal skills demonstrative of empathy and support.
  • Strong written and verbal communication.
  • Demonstrated ability to work collaboratively.
  • Demonstrated ability to work with families who face exceptional challenges.
  • Solid Microsoft Office skills.
  • Bilingual (English/Spanish) required.
Please note this position follows NYP's onsite vaccination requirements.

SALARY/


BENEFITS:

NMIC offers a competitive salary based on experience and education as well as a comprehensive and generous benefits package: medical, dental, life, and disability insurance are included; including 20 vacation days, 15 sick days, 5 personal days, 11 agency holidays, plus the week off from Christmas Eve through New Year's Day; a 401k with agency contribution; and other benefits.
