- Conduct admission assessments in line with guidelines and interview residents/significant others to complete health history.
- Assess residents with new, unfamiliar, or uncommon conditions on the unit.
- Interpret assessment data to detect acute psychological and/or psychosocial risks and document accordingly.
- Inform and educate residents on provided care or services.
- Contribute to a care plan based on existing problems, expected responses, and medical care plans.
- Communicate the care plan using action plans, goals, and timeframes.
- Required:
A valid Registered Nurse (RN) license in the State of New York. - Associate's degree or Nursing Degree from an accredited institution.
- Preferred:
1 year or more of RN experience in a skilled nursing setting. - Bachelor's degree in nursing.
