LPN Intake Coordinator
(full-time, direct hire position with excellent full benefits/bonus package in addition to a very competitive base salary structure)

Responsibilities:


  • Process all referrals for Medicare certified home health services
  • Qualify all intake requests for both clinical appropriateness and financial reimbursement
  • Ensure all intake data is accurate
  • Articulate an understanding of Skilled Home Health services in all communication with referral sources and team members
  • Exhibit excellent customer service
  • Work closely with scheduling, Billing, and Quality departments

Qualifications:


  • Licensed Practical Nurse
  • Minimum 1 year clinical experience
  • Proficient computer skills with data entry
  • Home Health experience preferred not required

Pay:
$65, $73,000.00 per year
