*- The Cancer Center & breast care client navigator's primary focus is to provide seamless integration of cancer care in a multidisciplinary setting. The navigator is a liaison for patients, families, direct care providers, and ancillary clinics across the facility, community providers, and the health care system to ensure continuity of care. The client navigator is based in the Bellevue Cancer Center, a multidisciplinary center affiliated with NYU Perlmutter Cancer Center and serving a population of medically underserved patients.
AREAS OF RESPONSIBILITY 1.
Under the direction of breast service providers, assists with coordination and continuity of care:
a) Serves as point of contact for patients and physicians for incoming/outgoing referrals b) Guides patients transferring to Bellevue on how to obtain and submit outside medical records, including pathology slides if available.
c) Orients new patients to clinic and Bellevue as needed, provides support and concrete services d) Facilitates interactions and communication with health care staff and providers e) Schedules patients for appointments f) Refers patients to financial counseling as needed g) Ensures patients are scheduled for appropriate follow up care and imaging h) Coordinates access to up-to-date cancer patient information (of all media) i) Tracks interventions and outcomes 2.
Connects patients to hospital, community, and social support services:a)
Identifies and mitigates barriers to care, in collaboration with Social Work b) Links patients to hospital and community resources c) Advocates for patients at Bellevue and the broader health care system d) Identifies and coordinates with sources of free resources for Bellevue patients (e.g, wigs, educational materials) Page 2 of 2 3.
a) Following up on providers' orders:
schedules mammograms, breast ultrasounds, and other imaging appointments. b) Facilitates linkage to MRI guided biopsy appointments at NYU Langone. c) Facilitates appointments to other Bellevue services as needed. 4.
Coordinates screening and outreach programs:
d) Organizes breast cancer month (October) and mothers' day (May) screening and outreach campaigns e) Liaisons with providers, radiology/mammography, and public affairs to organize events f) Reports to Breast Program Leadership (BPL) meetings on outreach data and screening outcomes g) Participates in other outreach events as appropriate 5.
Conducts patient satisfaction surveys a) Calls patients to conduct surveys by phone to gather breast patients' feedback about their care and experience b) Coordinates with other staff conducting surveys c) Records responses and commentsEXPECTATIONS - Uphold the mission, vision, and values of the facility. - Collaborate with all levels of staff to ensure the highest quality of patient care and service delivery. - Meet all compliance standards/requirements, including annual employee health evaluations, annual in-service, fire safety, etc. - Adhere to time and leave policies and procedures. - Maintain an appropriate and professional appearance.
