We are currently seeking a


Social Counseling / Care Transition Specialist (f/m/d):
initially on a fixed-term basis, with a good chance of extension.


Your responsibilities:


  • Implementation and assurance of patient discharge management according to § 39 para. 1a sent. 10 SGB V
  • Assurance of cost coverage procedures and initiation of coverage procedures
  • Social counseling
  • Identification and organization of posthospital care needs (outpatient and inpatient care, rehabilitation, etc.)
in close cooperation with wards and departments
Your profile:


  • Degree in social work (social pedagogy, social work) or education or training as a nursing professional with additional experience in social counseling / care transition
  • Structured and independent work style
  • Strong cooperative and communication skills
  • Methodological and pedagogical skills and competencies
  • Knowledge of social law as well as medical and nursing knowledge
  • German language skills at least at C1 level according to GER
  • Good MS Office skills (Word, Excel, PowerPoint, Outlook) and affinity for digitalization
