Schedule/Location:


Our Transgender Health Program is embedded in an ambulatory care practice located at 900 Cummings Center, Suite 107T, in Beverly.

We are seeking a full-time, LICSW to support this office in a hybrid capacity with 3 days onsite and 2 days remote (Thursdays on-site is a requirement due to clinic hours).


Due to the hybrid nature of the role, we ensure that our employees receive required technology and training to be proficient and independently productive in all job responsibilities regardless of work location.

Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.

The Clinical Therapist is an integral member of the NSPG Transgender Health Program (TGH) and works closely with the clinical and administrative staff to enhance the delivery of patient care services along the continuum of care.

This clinician identifies high-risk psychosocial factors of patients, provide therapeutic support and help specialty and primary care staff working within the TGH program understand the influence of those factors upon the course of medical care and assumes care coordination responsibility of the patients.

In some cases, and as appropriate, the Clinician may provide direct treatment/intervention to patients and families.

In other cases, the Clinician may work with the treating outpatient mental health clinicians, within and outside of our system, helping to ensure that treatment is focused and effective.


Upon referral from a team member, this Clinician assesses and evaluates a patient's psycho-social needs and/or services as the lead care coordinator to manage patients.

This position requires a high degree of leadership, flexibility, independence, and teamwork. This Clinician must have strong communication and be an integral team player.

This includes the ability to engage easily with patients, caregivers, ambulatory practices staff, community resources, and the TGH program Team.

This Clinician may work with the care team to ensure patients are receiving the highest level of care, at the right time in the right setting, achieving an optimum quality of life for the patient, and reducing overall costs of care.


Qualifications

  • Master's degree in social work required.
  • Current licensure in the Commonwealth of Massachusetts, Licensed Independent Clinical Social Worker (LICSW) required.
  • Minimum of 2 years of clinical experience working within the Transgender Health and LGBTQ2+ Community preferred (preferably postmaster's experience). Other experience may include postacute care settings or care coordination case management experience.
