  • Valid HHA certificate _(must have worked in NY within last 2 years)_
  • Complete physical _(within the last 6mo)_
  • PPD or QuantiFERON _(within 1yr)_
  • Rubella/Rubeola Vaccines
  • MMR titers
  • Drug test _(within 6mo)_
  • 2 reference letters _(Personal or Professional)_
  • Valid ID and Social Security Card
