    About Your Work

    The Health Navigator (Ambulatory Care) assists members in navigating and connecting to clinical and community resources. Supports clinical staff in identifying and providing outreach, and baseline assessment services to members. Establish and maintain effective, ongoing relationships by facilitating communication and coordination with members, their caregivers, PCPs, and other providers. Connects members with health screening programs, and other resources. The Health Navigator will focus on care coordination and short-term case management for members needing support in accessing medical, social or behavioral services.

    You will focus on:

    • Under general supervision and guidance from the CM leadership team, the Navigator supports clinical staff through the completion of components of case management and disease management programs.
    • Assist members in getting appointments & access to appropriate health care and community programs.
    • Provide one to one guidance, support, education, coordination of care and other assistance to members as they move through the healthcare continuum.
    • Initiate follow-up to confirm and coordinate additional needs.
    • Participate in case conferences and meetings with the CM team and medical directors to support effective care coordination.
    • Effectively establishes rapport with team members, patients/families, and physicians.
    • Ability to provide a high level of customer service to members, internal and external customers in a professional, service-oriented, manner
    • Educate and answer inquiries from members and or their caregivers about benefits, services, eligibility and referrals with a positive and professional approach, promoting member satisfaction and retention.

    The Skills, Experience & Education You Bring

    • High school diploma or equivalent education, experience or training required.
    • Minimum Experience: two (2) years in an acute care facility, preferably case management or emergency department.
    • Strong organizational skills.
    • Strong verbal and written communication skills.
    • Ability to effectively cope with stressful situations, manage, multiple, and sometimes conflicting priorities simultaneously.
    • Knowledgeable of managed care, including contracted and non-contracted providers.
    • Spoken and written fluency in English.
    • Maintains confidentiality of all PHI in compliance with state and federal law, and BTP policy.

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
