The Arbors Assisted Living is seeking a highly motivated, professional and compassionate individual to join our team in the capacity of Assistant Wellness Director. This individual must have a heart and compassion for seniors.
- The Assistant Wellness Director is responsible for assisting the Wellness Director with the supervision and coordination of all resident care and services on a 24/7 basis. This includes, but is not limited to assisting with activities of daily living and medication management. _
Qualifications
- High-School diploma
- Successful completion of a New York State Department of Health training program as a Certified Nurse Aide; Personal Care Aide; Home Health Aide OR Licensed Nurse OR documented assisted living/group home personal care training program approved by Regional Director of Healthcare
- Certification as a Medication Technician by Arcadia Management, Inc.
- Certification as a Supervisor by Arcadia Management, Inc.
- First Aid Certification
- Must be able to read, write and speak English
Experience
- Two years of related work experience in health care field
- Strong computer skills including Microsoft Word and Excel
Physical Requirements
- Ability to move intermittently throughout the workday, inclusive of sitting, standing and bending, pushing, pulling or moving of objects and residents.
Performance Responsibilities
- Assists the Wellness Director with supervising the overall functioning of the Wellness Department
- Works in conjunction with Wellness and Case Manager to coordinate resident services.
- Assists the Wellness Director with the supervision of the Wellness Department including hiring and scheduling staff; developing and disseminating daily assignments; and evaluation staff job performance as well as competency.
- Assesses resident on movie in including wellness assessment, checking physician's orders, transfer and premovein information.
- Communicates with physician, pharmacy and family member/resident's representative as needed.
- Informs physician of any change in resident's condition.
- Takes and carries out physician's orders.
- Follows up with physician's orders, examinations, services, testing, and laboratory workups.
- Assesses continuously the physical and mental needs/conditions of all residents and is knowledgeable of resident's condition at all times.
- Responds to emergencies; assures the resident's comfort and safety; secures necessary emergency medical assistance; calls physician; notifies family; arranges transportation to a medical facility/hospital.
- Completes resident incident reports as needed and reviews incident reports in the Wellness Director's absence.
- Monitors resident medications and ensures that medications are ordered and received in a timely manner.
- Monitors medication records to assure accuracy.
- Reads, initials and monitors resident log daily.
- Assists the Wellness Director with reviews of the resident's ability to selfadminister medications.
- Notifies the prescribing physician and Wellness Director of any changes in the resident's ability to selfadminister medications.
- Notifies the prescribing physician of any changes in a resident that appears unable to independently administer medications, and then notifies the Wellness Director and Case Manager.
- Communicates with primary physicians within one business day to advise of any change in medication/dosage prescribed or discontinued by another healthcare provider.
- Documents in the resident's record resident changes and events, response and results of response including but not limited to emergency assistance, due to illness/injury; change in resident physical/mental status; contacts with physicians, consultants, resident's representative; services provided; etc.
- In conjunction with the Wellness Director, oversees the resident's monthly weight recording program and ensures the Director and Doctor are notified when a resident either gains or losses more than five pounds in a one month period or trending over a short period of time.
- Completes weight audits for quality assurance.
- Assists the Wellness Director with other quality assurance measures by completing weekly and monthly audits.
- Communicates resident concerns/issues with Wellness Director, Case Manager, Attending Physician, Executive Director, family/resident's representative, and other departments, as needed.
- Ensure resident follow up medical visits are being completed.
- Ensures completion of resident's annual medical evaluation and mental health evaluation, if needed.
- Demonstrates appropriate interactions and positive relationships with residents and their families treating all with dignity and respect.
- Supports each resident's right to self determination; documents resident refusal of services offered; outlines for the resident, in conjunction with Case Manager and physician when necessary, possible implications of the refusal; informs resident,
