Shift:
Full Time Days 7AM-4:30PM when in outpatient clinic and 7AM-7PM when inpatient.


You Will Be Responsible For:


  • Utilizes sensitivity, responsiveness, caring and compassionate attitude with patients, families and visitors by listening and communicating effectively with courtesy, consideration and tact.
  • Facilitates and coordinates evaluations for potential organ recipients or living donors which includes, but is not limited to the following:
  • Reviewing patient medical records/data and identification of co-morbidities/ contraindications to transplant (or living donation)
  • Evaluates and treats patients with acute, chronic complaints and health maintenance concerns which include the following:
  • Assessment of patient health status through physical exam, reviewing patient history, diagnostic tests and procedures and reviewing labs and other critical data.
  • Diagnosis of acute and chronic conditions affecting all organs systems
  • Initiating orders for medications, laboratory and other diagnostic testing and medical treatments on appropriate forms
  • Attends and participates in transplant team meetings including Patient Review Committee, hospital interdisciplinary rounds, discharge planning and QAPI. Communicates with the transplant team in a professional and effective manner.
  • Observes, monitors, assesses and documents organ function and general health status as defined by program protocols.

This includes, but not is not limited to:

  • Writing and/or dictating progress notes, procedure notes, history and physical examination, transfers/discharge summaries in accordance with medical policies. Documenting, coding and completing diagnosis of patient's condition to facilitate appropriate insurance reimbursement.
  • Coordinating information flow among medical, ancillary, nursing, financial and insurance personnel

Job Summary:


The APRN supports the clinical operation of the Transplant Institute with primary focus on clinical services in the inpatient and outpatient setting.

Delivery of patient care is conducted in collaboration with the transplant surgeons and physicians including, but not limited to, physical exams, diagnosis and treatment, ordering and interpreting tests, counseling on preventative health, writing prescriptions and assisting in surgery.


In addition, the APRN/PA is responsible for coordinating and managing the patient care of high-risk patient populations throughout the continuum to optimize the patient's clinical, functional and psychosocial status, to provide the communication link for coordination of care among the multidisciplinary healthcare team involved with this patient type, and to orchestrate a performance improvement program that focuses on improving patient outcomes.


Under administrative guidance and physician supervision, provides comprehensive primary care services, acute care services and extended care services in the area of nursing and/or medicine for individuals and their families.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.


Qualifications:

EDUCATION AND EXPERIENCE REQUIRED:


  • A minimum of three years of experience in a large hospital or clinical setting

EDUCATION AND EXPERIENCE PREFERRED:


  • If Nurse Practitioner
  • Doctorate of Nursing Practice (DNP)
  • Liver/Hepatology Experience
  • 1 year of Inpatient Experience
  • 1 year of ICU experience

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:


  • Current BLS and ACLS certification
  • If Nurse Practitioner:
  • Current, valid State of Florida license as an Advanced Practice Registered Nurse
  • National Nurse Practitioner certification by either:

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

Clinical Coordinator Transplant Certification (CCTC) by the American Board of Transplant Certification (ABTC),
or
Certification from a clinical research professional organization, i.e., SOCRA, ACRP
or
Certification from the America Association of Heart Failure Nurses (AAHFN)

  • If Nurse Practitioner
(applies only to NP), certification issued by either:

  • the National Certification Corporation (NCC)
    or:
- the Pediatric Nursing Certification Board (PNCB)
