Provide case management services to individuals with developmental disabilities
Develop and implement individualized service plans
Conduct assessments and evaluations to determine client needs and goals
Coordinate and monitor services provided by a team of support staff
Assist individuals in accessing community resources and services
Advocate for the rights and needs of individuals with disabilities
Provide direct care and support as needed, including assistance with activities of daily living
Administer first aid and basic medical care, including taking vital signs
Maintain accurate and uptodate documentation of client progress and services provided

Bachelor's degree in a related field (e.g., social work, psychology, special education)
Experience working with people with developmental disabilities preferred
Knowledge of case management principles and practices
Ability to lift heavy objects and perform physical tasks as required (e.g., assisting with transfers, lifting)
Familiarity with home care services and regulations
Strong communication and interpersonal skills
Conflict management skills to effectively handle challenging situations
Ability to work independently and as part of a team
Valid driver's license and reliable transportation
