patient home visits on the far NORTH SIDE of Chicago

Home Health Nurse RN Responsibilities:

  • Completing nursing process in delivery of patient care which includes: assessment, planning, implementation, and evaluation of patient outcomes.
  • Total oversight of patient from admission to discharge.
  • Developing plan of care based on findings.
  • Serving as patient's advocate and maintaining ongoing communication with physician.
  • Timely completion of all skilled nursing documentation

Qualifications:

  • Current Illinois State RN License
  • Valid Drivers License and Reliable Vehicle

MINIMUM of 2 years experience as an RN:

  • Must be RELIABLE and DEPENDABLE
  • VALID CPR CERTIFICATION
  • Must be COMPASSIONATE when engaging with patients in their homes.
