Acenda Integrated Health is recruiting for a Case Manager to support the Integrated Case Management Services Program in Cape May, New Jersey.

_This position includes the option for a Hybrid work schedule._

Key Responsibilities

  • Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community
  • Provides postdischarge followup to support the consumer with community integration
  • Conducts assessments within correctional facilities and attends court hearings to advocate for persons served

Requirements:


  • Minimum requirement is Bachelor's degree in psychology, social work or a related field, plus 2 years of relevant experience.
  • Professionalism and a commitment to excellence in care
  • Must have and maintain a valid driver's license with an acceptable driving record.
