Your Role

As the Community Support Specialist, you will work as part of the Community Support Team (CST) to provide support services to individuals assigned to CST in order to decrease crisis episodes and increase community functioning, assisting consumers to achieve recovery goals.

This position conducts assessments and develops treatment plans in collaboration with the consumer, other CST members, the mental health team, other providers, and other parties the consumer identifies as important to treatment.

This position completes all required documentation in accordance with the standards of Rule 132/140, payers, other applicable licensing and accreditation bodies, and Sertoma Star policy.

This position is required to provide on-call support on a rotating schedule.


A minimum of 60% of Community Support Team services are delivered off-site in community settings and the consumers' natural environments.

Staff utilize vehicles provided by the organization to provide services in community settings.


Responsibilities

  • Through a team approach, provide direct services to individuals assigned to the CST, at least 60% offsite during times and locations most convenient to the consumer, in accordance with the individual's service plan. Complete all required documentation according to Sertoma policy, standards of Rule 132/140, Attachment B, payers, and accreditation bodies.
  • Complete all necessary assessments including the IATP, LOCUS, and required documentation, including billing notes, in accordance with Rule 132/140 standards.
  • Develop and implement crisis plans and treatment plans outlining treatment goals and interventions in collaboration with the consumer, CST, other providers, and other parties as identified by the consumer, following the standards of Rule 132/140, payers, and accreditation bodies.
  • Collaborate and effectively communicate with CMHC staff to ensure coordinated care, and to provide referrals for consumers no longer eligible for CST.
  • Provide direct CST services that promote the reduction of crisis episodes and increased community functioning in order to meet rehabilitative, resiliency, and recovery goals.
  • Maintain records of individuals on the CST in accordance to Sertoma Policy, Rule 132/140, Attachment B, payers, and accreditation bodies. Assist CST Lead in conducting regular quality assurance of CST records.
  • Participate in weekly team meetings, maintain current knowledge and understanding of the treatment, progress, goals, and needs of each consumer on the CST.
  • Maintain open and effective communication with agency and provider staff, family, and others in response to client's identified needs, client assistance/service needs, treatment plan followup, and input on adaptations needed to enhance progress toward goals. Work cooperatively with staff to ensure appropriate access to services as identified during the treatment planning process.

Qualifications

  • Bachelor's Degree in human services related field OR equivalent of Mental Health Professional (bachelor's degree in any field plus 5 years minimum of supervised experience in mental health or human services field).
  • Minimum of one year experience working with individuals with mental health and/or substance use disorders required.
  • Effective communication skills, both oral and written.
  • Valid driver's license and documentation of current auto insurance, with an acceptable motor vehicle report.
