Support Coordinator I - Palm Springs, United States - Desert Oasis Healthcare
Description
Provides clerical support and administrative coordination for the Intense Case Managers (ICMs) and acts as liaison between providers, ICM team, and patients/families to facilitate continuity of care and referrals to appropriate community resources.
- Documentation of members referrals, communication with member and family, and the communication with associated team members in the appropriate Electronic Medical Record.
- Maintain a work load that meets the needs of the organization while still giving the members the individualized attention that is needed. This is to be determined by both the SC I and the supervising manager. These criteria will be dynamic and may change based on the needs of the organization and the skill set of the SC I.
- Identify those members that have the potential for reutilization. When identified using the structure within Clinical Services referring those members to appropriate level of care or program.
- Obtain necessary medical records, coordinating appointments, diagnostic tests, and/or other needed information to ensure a smooth transition from one level of care to another
- Assist member with additional benefits from either the health plan or directly from Desert Oasis Healthcare that may include but not limited to transport, community resources, etc.
- Communicate and follow up with those primary and specialty medical offices that are involved in the care of those members that are presently being followed by the SC I.
- Assist as a liaison between the member/family and those within multidisciplinary team that may include: Primary Care Provider, Specialty, Telephonic Case Manager, Social Services, Home Health, Palliative Care or others within the realm of DOHC.
- Document within the appropriate Electronic Medical Record and ensures that the documentation meets the needs of both DOHC and of the associated health plan.
- During engagement with members addressing those criteria that ensure quality outcomes. These may include but are not limited to Quality Measures, Preventative Care, Routine Follow-Up, and Immunizations. These criteria may be met during Primary Care Provider visits, Annual Wellness Visits, Optometry Visits, and/or Wellness Fairs.
- Perform other duties as assigned.
- High school diploma or equivalent required. A minimum of 2years college education preferred.
- Previous 2 years experience in the medical field, preferably in Home Health or discharge planning.
- Familiarity with medical terminology.
- Strong computer ability.
- Must possess excellent telephone, and people skills, with an ability to interface with DOHC staff and outpatient providers in a pleasant and professional manner.
- Must be detail oriented and able to continually reprioritize multiple tasks.
- Bilingual preferred.
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
