JOB SUMMARY AND PURPOSE


Case Manager, AOD II, assesses participants and coordinates access to medical, educational, social, prevocational, vocational, rehabilitative, or other community services.

The Case Manager is the primary point of contact to coordinate services and care for assigned clients. In collaboration with the AOD Counselor and Therapist, the Case Manager ensures that critical linkages to support services meet the medical, mental health, housing and education needs of the participant are consistently and effectively delivered


DUTIES AND RESPONSIBILITIES

  • Conducts comprehensive screenings and assessments with individuals (and families as appropriate) to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information, as appropriate
  • Assess barriers facing the clients and develop a case plan.
  • Coordinate individualized planning with clients to meet short
- and long-term needs.

  • Provide service linkages and support systems to ensure identified needs are met.
  • Coordinates case conferences with clients, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.
  • Acts as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitates support groups and client participation
  • Network and communicate with referral agencies and community resources, as well as family members and natural supports
  • Responsible for transferring benefits from previous county of residence to Los Angeles County for beneficiaries who have moved
  • Facilitate necessary transitions in substance abuse treatment between levels of care (from residential to outpatient treatment, etc.)
  • Trains and Guides colleagues who are earning their AOD Certification (employees with current AOD Registration who are completing required supervised hours).
  • Other duties as may be required to meet program goals, and the goals of program participants

Qualifications:


REQUIREMENTS:


  • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications

EDUCATION:


  • Holds an Alcohol and Drug Counseling (AOD) Certification from an approved accrediting organization (CCAPP, CADTP or CADEE)

EXPERIENCE:


  • Minimum 1year general experience in case management

PREFERRED QUALIFICATIONS:


  • Bachelor's Degree, or 4 years of relevant experience
  • 2 years' experience working with target population
  • Knowledge of/experience with substance abuse services in L.A. County;
